luci-app-ddns: adaption to new luci structure and cleanup

- adaption to LuCI's new directory structure on github
- adaption to LuCI's new Makefile optional settings
- remove CHANGELOG from github
- directory structure cleanup
not all patches of 2.1.0-3 and before were applied to new directory
structure
